What is Adult Foster Care?
Adult Foster Care (AFC) is a MassHealth program that allows family members or trusted caregivers to provide daily care to an eligible individual living at home.
This program allows individuals to remain in familiar surroundings while receiving assistance with daily activities.

Who May Qualify?
To qualify for the AFC program, the member typically needs to meet the following criteria:
Age Requirement
Must be at least 16 years old.
MassHealth Coverage
Must have MassHealth Standard or CommonHealth.
Daily Living Support Needs
Requires assistance with activities of daily living.
Caregiver Availability
Must live with or have access to a caregiver who can provide support.
