Unlike traditional institutional settings, AFC emphasizes daily supervision, personal care assistance, and companionship. It is not 24-hour skilled nursing, but rather a community-based solution that ensures safety through professional oversight while keeping families together.
To qualify for the AFC program, the member must meet specific criteria set by MassHealth. Here is a quick checklist to see if you or your loved one might qualify.
Must currently live in Massachusetts.
Must be eligible for MassHealth Standard or CommonHealth.
Require assistance with Activities of Daily Living (bathing, dressing, etc.).
